About the job

Citizen Team Rønde seeks 2 social- and health care assistants on 37 hours/week or as agreed, partly to perform health-related tasks and partly to work on quality and improvement tasks.

The council of Syddjurs Municipality has chosen to strengthen nursing in citizen teams with 5.5 million DKK. This gives us really good opportunities to build a forward-looking culture where quality and improvement work is prioritized, so that the quality of citizen pathways is strengthened. This is entirely in line with the proposal now available for quality standards for municipal nursing and basic palliative care. Therefore we now want to put action behind the intentions.

Citizen Team Rønde is newly established, through a merger of teams. This means that we start by being together in shaping the forward-looking culture, and that we incorporate new working procedures where we through triage and a focus on strengthened health professional competencies ensure focus on citizens’ rehabilitation potential. It also means that you as a social- and health care assistant will take a large responsibility in nursing-related task resolution, and in contributing to the management of complex social-care pathways.

Quality and improvement work must be strengthened. We do this concretely by planning dedicated hours for this. As a social- and health care assistant in Citizen Team Rønde you will have 8-10 hours every 2 weeks to work on quality and improvement tasks within your own practice.

This means that you do not participate in the concrete task solving on these scheduled quality work days, but here you work with, for example, quality assurance of records and medication tasks, training and follow-up days with colleagues, teaching in the team, preparation of care pathway follow-ups, etc. You may also have the opportunity to work from home on your quality days.

We would really like you to

  • Be trained as a social- and health care assistant
  • Work systematically with a perspective on quality and improvement work in practice.
  • Have an overview and be able to coordinate the social- and health care efforts around the citizen
  • That based on health and elder legislation you can assess, triage, perform and document the citizen’s health and functional level.
  • Take responsibility for multidisciplinary discussions of health-related issues.
  • Assume the role of the care pathway lead.
  • Have a desire to work with quality and to take both health and social professional responsibility in our team development.
  • Be an inspiration to your team, and generously share your surplus and your knowledge.
  • The positions are primarily daytime shifts with weekend shifts every 3rd weekend

Our grass is green because we can offer you

  • A everyday life with close proximity, and flexible leadership, where the starting point is to ensure the team’s well-being and employees’ work–life balance.
  • Scheduled time for quality and improvement work in your own practice.
  • Flexibility in start times
  • A clear professional career path, where together with you we look into your career aspirations, e.g. continuing education, leadership, training supervisor, etc.
  • An engaged and professionally educated citizen team, primarily consisting of trained social- and health care helpers, social- and health care assistants, and nurses. In addition, there is a cross-functional quality-responsible nurse whom you are linked to.
  • We work with transparency, equality and psychological safety in the team, and we achieve results because we dare to use each other’s professionalities and are curious about one another.
  • Lots of cake, coffee and good moments – and we dare to stand by what we promise. It’s not a joke that we offer you time to delve into quality work, we see this as a must.
