Would you like to help develop the outward-facing hospital?

We are seeking two specialists in internal medicine: geriatrics or another relevant specialty, who are eager for both clinical work and to help develop new ways to create cohesive patient pathways across the hospital, municipality and the patient’s own home.

The health service faces a significant development task in the coming years. The implementation of the health reform will place additional focus on the coherence between hospital, general practice and municipal services and on more patients being able to receive treatment closer to their daily lives.

At Medicine 2 we see this as an important clinical and organizational task. We are launching several initiatives across the department’s functions, which collectively support the development of the outward-facing hospital. We now want to strengthen this work with two specialists who will help drive the clinical task and develop the new collaboration forms.

A clinical function with a clear development task

A substantial part of the position will be medical tasks at health and care placements (SOP), integrated with the department’s other duties involving geriatric patients, the multimorbidity unit and home hospitalizations. The position is therefore relevant for you who are interested in clinical patient pathways at the interface between the hospital and the local health care system.

Not all solutions are fixed yet. Part of the task will be to develop them together with the department’s management, colleagues and partners. Central questions will include:

• How do we create coherence between SOP, home hospitalization and the hospital’s other clinical functions?
• How do we ensure high medical quality when treatment takes place across sectors?

A position with room for your professional profile

The task in SOP and in the outward-facing hospital combines with clinical functions in Medicine 2. Depending on your interests, experience and competencies, it could for example be within:

• geriatric ward and outpatient clinic
• Diagnostic Unit
• Unit for multimorbidity
• home hospitalization

There are also considerations about establishing a polypharmacy outpatient clinic if the right professional profile and interest are present. The position can thus be composed so that you can both have a clear role in developing the outward-facing hospital and at the same time maintain or develop a clinical area of interest.

About Medicine 2

Medicine 2 has 70 beds with its own on-call and back-up on-call team. The department houses several internal medicine functions, and at Holbæk Hospital there is close collaboration between the medical specialties.

We want to actively use the breadth of the department. The ambition is increasingly to think of the department’s various initiatives together in patient pathways that can move more flexibly between the hospital and services closer to the patient. Innovation, digitalization and cross-sector collaboration are therefore part of the clinical development and not a separate track alongside operations.

Geriatrics

Geriatrics at Holbæk Hospital consists of a 20-bed ward, a geriatric outpatient clinic and an ortho-geriatric function. The ward mainly receives patients from the Acute Department with geriatric problems and has integrated potential for rapid follow-up after discharge in our flexible discharge clinic.

The geriatric outpatient clinic provides broad geriatric assessment and follow-up of patients with multimorbidity, age-related issues and functional decline. The outpatient clinic also has a regional function for complex falls, syncope and dizziness, with specialized assessment and multidisciplinary conferences in collaboration with, among others, cardiology, neurology and ENT.

The ortho-geriatric function supports the postoperative course for patients over 65 years admitted to the orthopedic department with hip-related fracture.

You are a specialist in internal medicine: geriatrics or another relevant specialty. We emphasize that you:

• Have an interest in patients with complex and multi-faceted needs.
• Are curious about new ways to plan treatment and follow-up.
• Thrive in cross-disciplinary and cross-sector collaboration.
• Want to contribute to collegial sparring, supervision and education

You do not need to be an expert in cross-sectoral functions or home care from the outset. The most important is solid medical expertise combined with an interest in helping to develop the area.

Professionalism, education and work environment

We place emphasis on high professional standards and a mutually respectful work and learning environment. There is teaching for the doctors twice a week as well as weekly teaching in your own subspecialty. Geriatrics has a main training program in geriatrics, acute medicine, internal medicine common trunk and general practice. As a specialist you are also expected to contribute to the department’s professional environment, supervision and education.

Shift and employment conditions

The position can be part of Medicine 2’s on-call team. The specific work organization is agreed taking into account the function of the position and your profile.

Salary and employment conditions are according to the applicable collective agreement.

Starting salary for employees under a collective agreement

The region is obliged to inform you of the starting salary for the position. You will be told base salary, centrally set allowances and pension in the relevant collective agreement. In addition to the starting salary, the pay may include any negotiated allowances depending on the position, your experience and qualifications.
