About the job

Would you like to help develop the rehabilitative effort and create good, coherent processes for citizens?

Favrskov Municipality is in the process of establishing a new organization of the Authority for the Elderly, where authority tasks, professional mapping and rehabilitation are gathered in cross-disciplinary citizen teams.

We are looking for a health professional who is passionate about rehabilitation and about taking as a starting point the citizen's own resources, wishes and opportunities.

You will play a central role in our work with professional illumination and rehabilitation. You will both work directly with the citizen in concrete processes and use your rehabilitation expertise to advise and support employees in home care.

You will become part of a strong multidisciplinary environment, where we work closely together to create holistic processes and find the right intervention together with the citizen.

Your workday will be diverse and will partly involve working in a multidisciplinary team of colleagues, carrying out interventions in the citizen's own home and reporting weekly to the fixed team in home care.

Your tasks revolve around three main areas

Professional clarification of needs

You conduct processes with citizens where there is a need for closer professional clarification of the citizen's functional abilities, resources and needs before a decision on help can be made.

The aim is to contribute to ensuring that the citizen's needs are sufficiently informed, and that there is a professional basis for the further assessment and decision.

You should among other things

  • observe and test the citizen's functional ability in daily activities
  • identify the citizen's resources, needs and possibilities to cope more independently
  • include the citizen's own wishes and goals
  • document your observations and professional assessments so they can be included in the authority's overall information on the case and the basis for the decision.

Rehabilitation process

You plan and implement rehabilitation processes for citizens under 67 years old with focus on supporting the citizen in regaining or maintaining as much independence as possible in everyday life.

You should among other things

  • set goals for the process together with the citizen
  • design and carry out concrete rehabilitative interventions
  • work with the citizen's motivation, coping and own resources
  • follow up the process and adjust the effort along the way.

Rehabilitative effort in home care

You shall support the municipal home care staff in working rehabilitatively in processes under the Elderly Act. Here your role is advisory and supportive.

You should among other things

  • advise and brainstorm with staff about the rehabilitative approach
  • support staff in translating rehabilitative goals into concrete actions in the citizen's daily life
  • contribute with professional observations, methods and new perspectives when a citizen's process calls for it
  • collaborate with staff to maintain focus on the citizen's resources, motivation and ability to manage as much self-care as possible.

You will become part of the multidisciplinary collaboration in the Citizen Team and work closely together with among others case managers, home care and other health professionals.

We are looking for you who

  • have a health professional education, for example as an occupational therapist or physiotherapist or social and health care assistant with special experience and competencies in rehabilitation
  • have experience with or strong interest in rehabilitation, everyday life mastery and citizen involvement
  • can perform professional observations and assessments of the citizen's functional ability, resources and needs
  • can translate your observations and professional assessments into clear and usable documentation
  • are good at setting concrete and meaningful goals together with the citizen
  • can advise, motivate and create followership among other professional groups
  • thrive in a role where you work independently with your own citizen processes and collaborate closely with others
  • are curious, development-oriented and want to help shape our new way of working
  • have good oral and written communication skills.

The most important thing for us is that you have a strong rehabilitative perspective and can use your professional skills both directly with the citizen and in guiding other staff.

Therefore you should choose us

You will become part of an organization in development where you have the opportunity to influence the establishment of the new Authority for the Elderly. Together with skilled colleagues you will help create coherent citizen processes and develop a shared rehabilitative practice.

We offer

  • an exciting position with great variation between citizen contact, professional mapping, rehabilitation and guidance
  • opportunity to work targeted with rehabilitation both in your own processes and together with the municipal home care
  • great professional autonomy and opportunity to influence task solving
  • professional sparring with colleagues with the same task type and good opportunities for competence development
  • opportunity to influence the establishment of the new Authority for the Elderly.

Practical information

The position is essentially full-time with expected start on 1 January 2027. Your workplace is Hadsten Health Center, Hovvej 76, 8370 Hadsten.
