Nurse, Team Tilst, District North, Aarhus Municipality

About the job

Are you interested in professional development, cross-disciplinary collaboration and the good citizen journey, and do you have a curious approach to the connection between nursing, rehabilitation and holistic care in accordance with the Older Persons Act?

We are seeking a nurse who can take on the role of care pathway lead in rehabilitative processes, both under the Health Act, the Social Services Act and the Older Persons Act, where holistic care is in focus.

In collaboration with other health care professionals in Care and Rehabilitation, you will identify, assess and evaluate citizens in single or complex pathways due to newly arisen reduced function, increased treatment needs or needs for investigation or in connection with discharges. In addition, a nursing professional perspective through holistic care in long-term pathways.

Are you motivated to provide high-quality nursing and to ensure that citizens experience safety, continuity and presence in their pathways? You will have the opportunity to work closely with citizens and become part of a professional environment where collaboration, quality and development are highly valued. The nursing tasks span a broad target group consisting of both older and younger citizens with different needs, which gives you ample opportunity to bring your entire professional skillset into play.

The tasks are often complex and require both overview, engagement and a holistic view of the citizen’s situation.

In daily practice you will be a central professional actor in ensuring coherent pathways. You will work in close collaboration with social and health personnel, therapists and coordinating functions, and you will experience that your expertise is used, valued and sought after. We are deliberately working to create a culture where professional sparring, curiosity and openness are the driving forces in our joint development.

Your shifts will be day shifts with weekend shifts in a cadence we agree on together. You will have influence over the planning of your tasks in collaboration with the citizen-centered resource coordinator, and there are good opportunities for professional development and participation in relevant initiatives and pathways.
