Your daily life will be versatile with your primary function to

• Assess and provide clinical nursing care to citizens in complex and unstable pathways.
• Quality assure complicated discharges by coordinating and initiating ongoing cross-sector collaboration.
• Contribute to daily planning, lead and coordinate coherent pathways and triage at board meetings.
• Ensure that knowledge about complex pathways is anchored and supports our ongoing quality work.

As a nurse with a specialization in citizen-centered nursing you thrive on applying your professional skills when meeting citizens with diverse, complex and unstable issues. You are a professional role model and act as a resource person across sectors and professional groups.

You become part of Frederiksberg Municipality's beautiful areas, and you will find that even though we are very on the move, at the citizens' homes, in the nursing clinic, or performing screen visits, we meet several times a day in multidisciplinary teams and prioritize professional sparring.

The practical

You will primarily work day shifts with a few evening shifts and a weekend shift every 4 weeks. You are employed in Home Care area 10 and operate from Finsensvej 80B.

Your hourly employment and shift plan will be discussed collaboratively. It is required that you have a driver's license. Before your employment we ask you to present a private criminal record certificate.

You are covered by the municipal agreement with DSR, where a agreement has been negotiated specifically for nurses with citizen-centered education. In addition, your previous experience will be locally negotiated.
