About the job

Do you want to use your professional skills to create coherent citizen journeys and ensure the right help?

Are you an occupational therapist, physiotherapist or nurse? We are looking for two visitation officers for Visitation Health and Care, who will translate the values of the Social Services Act and the Elderly Act into practice based on the citizen's wishes, needs and resources, with a start on 1 January 2027.

We have received extra resources to strengthen presence in citizen-near teams. You will become part of a varied daily life with authority work, citizen contact and interdisciplinary collaboration.

About the position

You assess the citizen's need for help and support for an independent, meaningful and active everyday life. In dialogue with citizens, relatives and partners, you take your starting point in the citizen's overall situation, wishes and resources. You ensure legal protection and make decisions according to applicable law and Viborg Municipality's service level. The role requires you to combine your health-professional knowledge with clear communication and a secure professional judgment.

We work with self-determination, trust in employees, citizen-near leadership, rehabilitation and coherent pathways in coordination with relatives and civil society. As a visitation officer you contribute to translating these values into concrete solutions together with the citizen and other professionals. This entails both having an eye for the current help and for the citizen's possibilities to maintain or regain skills.

Your workday alternates between home visits, conversations, case processing and coordination with partners. You solve tasks both with citizens, in the districts and at Viborg Town Hall. Here, telephone and discharge shifts are part of the department's common tasks.

Your tasks will include

You must perform the authority function within personal care and practical help, temporary stays as well as elderly and nursing homes. The tasks range from the initial dialogue and mapping of the citizen's situation to decision, follow-up and revisitation.

  • assess the citizen's functional capacity, needs and resources through home visits and conversations with citizens and relatives
  • make and document decisions and handle complaint cases
  • participate in interdisciplinary meetings and provide sparring on legislation, the citizen's rights and the municipality's offerings
  • handle telephone duties and discharge duties with origin from Viborg Town Hall
  • revisit citizens in connection with new legislation
  • support good transitions and coherent citizen pathways.

We use KMD Nexus as the care system and document according to FSIII.

We are looking for a person who

You have a desire to work at the crossroads of professional assessments, legislation and people's everyday lives. You can listen and create a good dialogue, while being clear about the framework for help and the justification for your decisions. You thrive with both citizen contact and the written case processing that accompanies the authority role.

  • are an occupational therapist, nurse or physiotherapist, preferably with visitation education and/or experience as a visitation officer
  • can and will take on the role of an authority
  • have an interest in citizens with physical, psychological, cognitive and social challenges and value dialogue with citizens and relatives
  • bring your professional authority constructively into the interdisciplinary collaboration
  • work independently, collaborate well and plan flexibly with a view to the team's shared tasks
  • thrive with a varied daily life and administrative work and preferably have experience with a care system
  • communicate confidently and clearly both in writing and orally
  • want to contribute to the development of holistic care and coherent citizen pathways
  • have a driving license

With us you get

You will become part of a professional community where we use each other's knowledge and experiences and collaborate to solve the tasks. We emphasize that you settle into the role with a thorough introduction, a mentor and ongoing sparring opportunities. At the same time, you get space to bring your perspectives into play and contribute to the development of our practice.

  • a permanent position in a department in development
  • influence on professional development and the visitation officer's role in interdisciplinary collaboration
  • a varied work week with tasks both in the districts, at Viborg Town Hall and at citizens' homes

•

  • a thorough introduction to your tasks with an experienced colleague attached as a mentor
  • engaged colleagues and a good work environment with focus on professionalism, development, collaboration, well-being and mono- and cross-disciplinary sparring
  • opportunity to work self-planned while considering the department's joint tasks
