We are seeking dedicated and clinically strong nurses for new evening shift positions as outreach nurses in Sygepleje Vest, with primary affiliation to the nursing homes

You will become a central part of a new way of organizing work, which will help shape and pave the way to ensure a coherent, flexible and professionally strong nursing coverage at the area’s care homes in the evening hours. You will be responsible for the area’s care homes, which are the care homes in Stenstrup, Ollerup, Aldersro, Demensbyen, Tåsinge and Vindeby, where you contribute with nursing professional sparring, guidance and support to the staff on duty. You join the team of visiting nurses.

The positions have been created to foster a greater sense of community and to strengthen collaboration between care homes, home care and nursing, so that citizens experience high professional quality and continuity in their care pathways.

About the position

In the new evening shift position you have a central role in the nursing care efforts at the care homes and in collaboration with the visiting nurses. Your meeting place will be in the nursing department in Home Care West, where your nursing professional colleagues also report for duty.

You will be on duty with two other colleagues, who will be out in Home Care West.

You work independently and continuously coordinate with staff and nursing professional colleagues and managers. At the care homes there are fixed nurses attached during daytime on weekdays, with whom you will also have close collaboration.

You are responsible for, that

• place a high emphasis on collaboration with residents and relatives
• be an important professional support for staff and contribute to safety, quality and development in daily task execution
• handle planned, acute and outreach tasks
• perform nursing clinical assessments and prioritizations
• coordinate your own tasks in collaboration with the care homes
• document performed interventions according to current guidelines
• contribute to interdisciplinary and cross-sector collaboration between the care homes and the visiting nurses
• teach and guide staff on shift
• relate to incoming care plans and discharge reports

A strong cross-cutting collaboration

We want to strengthen the collaboration between the care homes and the visiting evening shifts to ensure coherent citizen pathways, high professional quality and a sensible use of nursing resources.

We have a close multidisciplinary collaboration with relatives, pharmacists, dementia consultants, specialists etc.

Nursing in Home Care West consists of 35 nurses, who are a good mix of age, experience and professional competencies. We are a well-functioning team with broad and high professional quality, where collaboration, sparring and togetherness are in the spotlight.

We have weekly cross-disciplinary meetings where we are a natural part of the overall assessment of citizens’ needs.

We focus professionally on the task-solving for citizens and the many partners around us. We work from a citizen-centered perspective and with a rehabilitative approach aimed at preventing hospital admissions.

We work with structured competence development, tailored to the individual employee’s wishes and professional interests. At the same time, it is a fixed part of employment that you complete a diploma module in rehabilitation and dementia.

In nursing there are various specialist functions including wound care, palliative care, Nexus, continence, clinical supervision and mentoring for newly employed colleagues as well as various key roles.

Salary and employment

Salary and terms of employment in accordance with the applicable collective agreement.

We expect our new colleague to start on 1 January 2027. The position is strictly evening shift with weekend work every second weekend.

The position is 37 hours, but we are flexible and will as far as possible accommodate your wishes regarding hours.

In Svendborg Municipality the salary for this type of position typically ranges between DKK 37,534 and 54,773 per month at full-time employment. The salary includes pension, special holiday allowance and fixed allowances. There may also be time-appropriate allowances and other special payments. It is important to note that your exact salary is determined by your experience, seniority and competencies and is agreed upon through negotiation with the relevant professional organization.

There must be presented a private criminal record upon appointment to the position.
