Coordinator for Home and Nursing Care Fjord

In Home Care and Nursing Fjord we are seeking a dedicated coordinator on 37 hours per week for a central role in our organization. The position is essentially with working hours Monday to Friday from 06:30 to 13:54.

We work according to the Sønderborg model, where fixed cross-disciplinary teams in close collaboration around the citizen are the foundation of our effort. We believe that the best solutions are created collectively, and therefore relationships, coordination and interdisciplinary interaction are essential elements of the role.

About the position

As a coordinator you will be an important team player in daily operations and planning. You will become the coordinator for:

• Team Rehab
• Specialist team
• Clinics

At the same time you will be part of a coordination team, where you collectively ensure coherence and coordination across the entire Home Care and Nursing Fjord.

You will play an important role in supporting our teams, creating an overview and ensuring that the right competencies are present around the citizens.

Your tasks

You will, among other things, work with

• Daily coordination and allocation of tasks.
• Interdisciplinary on-call and resource management.
• Planning in Cura Plan and Cura Production.
• Shared responsibility for the interdisciplinary on-call schedule in KMD.
• Close collaboration with management, staff and partners.
• Support for coherent citizen pathways.
• Contribute to the development and optimization of workflows across teams.

We are looking for someone who

• Has experience with coordination, planning or dispatching.
• Thrives in creating an overview and maintaining calm in a dynamic daily life.
• Can work in a structured way and prioritize tasks in an operation with many stakeholders.
• Can see the value in interdisciplinary collaboration and relational coordination.
• Communicates clearly, respectfully and acknowledgingly (the personnel policy values).
• Has a flair for digital systems and sees IT as a natural working tool in everyday life.
• Is motivated by data-driven work and using digital solutions to create overview, quality and coherence.
• Has experience with or interest in Cura Production, Cura Plan and KMD Vagtplan.
• Has knowledge of, or interest in, the Health Law and the Social Services Law and understands the significance of the legislation in the daily coordination of tasks and interventions.
• Preferably has experience from home care and nursing or a similar health and care field.
