About the job

Nurse with a desire for professional challenges and rehabilitation – become part of the Health Unit Tilst-Skejby

Do you want to use your nursing skills independently, work with complex citizen pathways, and make a noticeable difference in people's daily lives?

At the Health Unit Tilst-Skejby we are looking for a nurse who wants to put their professional expertise into practice in the citizen's own home. Here you will have a varied daily life with professional challenges, close interdisciplinary collaboration, and the opportunity to follow the citizen's development through a rehabilitative process.

You will become part of an engaged team where we prioritize professional sparring, psychological safety, and a good collegial community. We know that complex citizen pathways require both professionalism, overview, and collaboration – and that we succeed best when we help each other.

Does this sound like something for you? Then read on.

What does the job involve?

We work with shorter, targeted rehabilitation pathways, which typically last 8–12 weeks. Our goal is to support citizens in regaining functional ability, strengthen their coping, and achieve the greatest possible independence in daily life.

The target group is broad and includes both younger and older citizens with physical, cognitive and psychological challenges. Common to the citizens is that they, for a period, need a targeted effort where nursing, rehabilitation and interdisciplinary collaboration go hand in hand.

As a nurse you will have a central role in creating coherence in the citizen pathways. You should be able to see the whole person, identify needs, coordinate actions and follow up to ensure that we are moving in the right direction.

You will have independent, traveling visits during the workday and meet citizens with different and sometimes complex problems. It provides a varied daily life where you will use your clinical nursing expertise, your ability to build relationships, and your sense for finding solutions.

Your tasks

Your tasks will include, among others

  • Receive citizens and assess their needs for nursing, care and interdisciplinary actions.
  • Prepare, coordinate and follow up on action plans with focus on the citizen's goals and development.
  • Be pathway-responsible or contact person and ensure progress and coherence in the citizen pathway.
  • Provide instrumental nursing, including wound care, care for various drains, IV treatment as well as care and replacement of probes and catheters.
  • Guide and support citizens in self-care and in mastering everyday life as best as possible.
  • Provide palliative nursing to terminally ill citizens.
  • Provide nursing care to citizens with psychiatric issues and substance abuse.
  • Collaborate with colleagues, relatives and other partners on the best possible effort for the citizen.

The tasks require both clinical insight, independence and the ability to prioritize. You are not alone with the responsibility – we emphasize that we actively use each other and seek professional sparring when needed.

Who are you?

We imagine that you are a nurse who is curious about people, motivated by professional challenges, and would like to help create good and coherent citizen pathways.

You thrive on taking responsibility and working independently, but also know that the best solutions often arise through collaboration with others.

We especially emphasize that you

  • Are a trained nurse and have an interest in nursing in the primary sector.
  • Want to work rehabilitatively and base your work on each citizen's resources and goals.
  • Are good at building relationships and communicating with citizens, relatives and colleagues.
  • Can maintain an overview, prioritize and act appropriately when situations change.
  • Take responsibility for your tasks and follow up on agreements and actions.
  • Contribute actively to collaboration and can both offer and ask for help.
  • Desire to develop professionally and personally.
  • It is an advantage if you have experience from a similar role in the Magistrate's Department for Health and Care (MSO) and familiarity with Cura, but this is not a requirement. We offer an introduction and training based on your competences and experiences.
  • You do not need to be able to do everything from day one. The most important is that you want to learn, take responsibility and contribute to the community.
  • You must have a driving license and be able to cycle.

What do you get with us?

We want you to experience both professional development and job satisfaction in daily life. With us you get:

  • Professional challenges and variety: You meet citizens with different and complex needs and get the opportunity to bring your clinical nursing skills into play.
  • Independence and responsibility: You take a central role in coordinating citizen pathways and creating progress in collaboration with the citizen and the other professional groups.
  • Regular professional sparring: We hold mono-disciplinary nursing meetings every week where we share experiences and discuss nursing issues. In addition, we hold interdisciplinary meetings twice a week where we coordinate actions and create coherence in the citizens' pathways.
  • A safe work community: We prioritize professional discussions and psychological safety. You must be able to work independently, but you are not alone. We help each other, share our experiences and actively use each other’s professional skills.
  • A thorough introduction: You receive introduction and training based on your competences and experiences.
  • A good collegial community: We value both daily collaboration and the social community.

We are one consolidated health unit with two entry points in Tilst and Skejby. You must therefore be prepared to handle tasks and follow citizens across both teams.
