As a social and health care assistant you are the consistent professional in the stable citizen pathways. You create coherence between health-related interventions and daily care and nursing. In close cooperation with nurses, therapists and social- and health assistants you have a central role in planning, coordinating and following up on the actions around the individual citizen.

The tasks are many and include, for example

• To ensure high quality and coherence in citizens' care and treatment
• Identifying early illness and responding to changes in the citizen's condition
• Active participation in daily triage and other interdisciplinary meetings
• Whole-person care around the citizen
• Wound care, medication management, measuring values and other delegated tasks

The practical

Primarily day shift in the time frame 7-13 / 7-15

Weekend shift every 3 weeks

Opportunities for evening shifts if desired.
