Social and healthcare assistant for an outreach team at Birkebjergcentret, South-East District

We work in multidisciplinary composed teams that operate in mixed shifts, day and evening. In the team we are subdivided into mini-teams. To be able to handle increasing complexity and unpredictable daily life, we have citizen conferences and daily triage in the multidisciplinary mini-team, with a focus on rehabilitation.

About you: You are educated as a social- and health care assistant and eager to apply your professionalism and be willing to develop. You want a hands-on job where you give a lot of yourself but also get a lot in return, and you see collaboration across with the citizen, colleagues, relatives and other partners as a given. You have a driving license, so you can travel around the southern part of Næstved in any weather.

Salary and employment terms are in accordance with the applicable collective agreement between KL and FOA.
