Are you looking for a job where you can help citizens become more self-reliant in everyday life through training and rehabilitation?

We are currently seeking a colleague who will have a unique opportunity to help build our new DigiRehab initiative in Hvidovre.

About the position

As part of the DigiRehab team you will accompany the citizen through a 12-week training program with two trainings per week of approximately 20 minutes.

Your work will include screening the citizen, from which the DigiRehab app assembles individual training programs corresponding to the citizens’ functional level. You will therefore be responsible for the training in collaboration with the citizen, adapting the exercises to the daily form and following the development along the way.

The position is 37 hours per week with a focus on DigiRehab. Since there are not yet any training programs established, the initial duties will primarily consist of care tasks. The ambition is for DigiRehab to take up more and, hopefully in due course, fill the entire position. The training programs will be planned separately and therefore will not be carried out concurrently with care tasks.

You can look forward to a daily life with a lot of cross-disciplinary collaboration, both with occupational therapists, physiotherapist, the planner, other colleagues in DFS Plus as well as Hvidovre Municipality’s DigiRehab trainers.

Your primary areas of responsibility

• Screen the citizens in the DigiRehab app and set concrete and realistic training goals according to the SMART model
• Train with the citizens, motivate them and adapt the exercises to their functional level and daily form
• Follow up on the goals and document the training, the citizen’s development and your observations
• Be attentive to changes in the citizen’s condition and pass on relevant information to colleagues and partners
• In time identify citizens who may benefit from DigiRehab, among others through cross-disciplinary meetings and citizen lists
• Take care of care tasks to the extent they are part of your position

We expect that you

• Are educated social and health helper or social and health care assistant
• Have an interest in training and rehabilitation and in helping citizens cope more independently in everyday life
• Have an understanding of clinical reasoning and can use your professional knowledge to assess the citizen’s needs and adapt the training
• Work in a structured way and are thorough with documentation and follow-up
• Are good at creating a secure contact and motivating the citizen to participate in training
• Are prepared for that the job also includes care tasks, especially in the start, and that you can work with different citizens
• Have a driver’s license

Experience with DigiRehab is an advantage, but not a requirement. If you have worked with training and rehabilitation, for example as a training assistant, it is also relevant.

Is we your new workplace?

Your primary work area is in Hvidovre. If, for a period, there are not enough tasks in Hvidovre, you must be prepared to take care tasks in Ishøj, Vallensbæk, Rødovre and Brøndby.

Working hours are generally from Monday to Friday 7:00-15:00, no weekend work.

The care tasks are performed as a flying function . This means that you visit different citizens and are not necessarily tied to a fixed route.
