Botilbud Elbæk in Hadsten is seeking an engaged and professionally strong health professional coordinator who is passionate about working closely with both citizens and staff.

The position is a combined role, where in the mornings you help to support and accompany citizens in the transition to day care for 10-15 hours per week, and the rest of the working day you take on the tasks as a health professional coordinator. Working hours are on weekdays 7-15, with possible meetings in the afternoons and evenings.

About Botilbud Elbæk

Botilbud Elbæk is a dynamic and lively offering with space for 39 residents distributed over 2 groups. The target group is adults with developmental disabilities to varying degrees who need support and help based on an individual pedagogical approach. Some citizens also have diagnoses such as autism, ADHD and anxiety in addition to developmental disability.

Some of the citizens are more or less self-sufficient in relation to personal hygiene, dressing and eating, while others need help with all practical and personal tasks. In this context, transfers and personal care occur.

As a new employee at Botilbud Elbæk you will join an employee group consisting of experienced pedagogues, social and healthcare assistants, occupational therapists, social workers, psychomotor therapists, physiotherapists, care assistants, cleaning staff, group leaders and a department manager.

Our social-professional work is based on a rehabilitative approach, where we work from the citizens' individual action plans and sub-goals. Due to the citizens' functional limitations there is a sharp focus on health-related interventions, where in order to strengthen interdisciplinary collaboration, we wish to hire a health professional coordinator.

Your tasks

In the morning (10-15 hours per week)

• Support and motivate citizens in connection with morning routines
• Carry out care tasks for those citizens who need this
• Help citizens get off to a good start at day care
• Create structure, calm and security in the transition from home to day care
• Work closely with the rest of the staff regarding the citizens' needs

As a health professional coordinator (22-27 hours per week)

• Coordinate and ensure quality in the health professional interventions
• Support staff in health-related problems
• Ensure documentation and follow-up in relevant systems
• Contribute to the implementation of guidelines and procedures
• Collaborate with doctors, hospitals, relatives and other partners
• Participate in professional sparring and development of health professional practice
• Contribute to training and guidance of staff at Botilbud and Day care
• Prepare agenda for medication and staff meetings, have overview and delegate tasks

We are looking for you who

• Have a health professional education, e.g. nurse or social and health care assistant
• Have a strong health professional profile
• Are structured and can keep an overview in a daily life with many collaboration interfaces
• Have experience with or willingness to teach colleagues
• Have good collaboration skills and a desire to contribute to joint development
• Have experience with documentation and preferably familiarity with relevant IT systems

Salary and employment terms

Salary and employment terms will be according to the applicable collective agreement. Final permanent appointment takes place after negotiation with the negotiating union based on qualifications.
