Do you want to make a difference in everyday life – together with the citizen?

Sometimes development begins with an important conversation. Other times with support for a practical task, a trip into the city or with finding the courage to try again. As a contact person with us you help create the framework for people with mental health difficulties to live as independent, meaningful and valuable a life as possible.

We are seeking a committed contact person with a relevant social- or health-related education – e.g. pedagogue, occupational therapist, nurse or social- and health-care assistant. You do not need to know everything in advance. We place emphasis on professional curiosity, integrity and the desire to develop together with the citizens and your colleagues.

A job close to the person

Dannebrogsgade and Saxogade are a social-psychiatric residential facility for 47 adult citizens aged 32-70. The citizens live in their own apartments and have mental illnesses – several also have physical diagnoses and/or an active use of substances. The service is for citizens referred under the Housing Act § 105 and Social Services Act §§ 83 and 85.

We work in the citizens' own homes and in their local environment. This means that the relationship, trust and the individual goals are the core focus – and that everyday life can differ from citizen to citizen. We support without taking over and work to enable citizens to develop strategies, strengthen their independence and preserve hope, even when the road ahead is not straight.

How we work

Our professional starting point is recovery and rehabilitation. In practice, we draw on, among other things, motivational interviewing (MI), Low Arousal, LA2 and dialogical practice. We are also exploring how nature can have a larger role in the collaboration with the citizens – both by going out and by bringing nature into daily life.

You will become part of a stable employee group with 14 contact persons with a social- or health-professional background. We divide ourselves daily across the two addresses and work closely with among others psychiatrists, nurses, substance misuse consultants and caseworkers. In addition, there is a social-professional service leader, a health-professional department head as well as students and on-call substitutes.

Your tasks as contact person

You will be attached to a fixed team and have contact person responsibility for three to four citizens. Your tasks will among other things include

• build trustworthy relationships and support the citizens in articulating and working with their own goals
• support the development of strategies to cope with mental health challenges and possibly substance use
• create opportunities for a more independent and meaningful everyday life – including activities and participation in the local community
• conduct conversations, accompany the citizens outside and support practical tasks in their home
• support the citizens' mental and physical self-care and carry out relevant health-professional tasks
• work with documentation, evaluation and medication management
• collaborate cross-disciplinary and actively contribute to professional reflection and development of practice

You will receive a thorough introductory program, so you can learn the tasks, the citizens and the colleagues. We prioritise daily sparring, regular team meetings, supervision and teaching from external actors. We place high value on competence development and a good working environment.

Employment terms

Salary and employment according to the current collective agreement and within the framework of local salary formation. We will obtain references and a criminal record certificate in connection with the recruitment process.

Working hours 37 hours per week and located in the time frame 8.00-18.00. You work every third weekend 8.00-16.00, and there is also staffing on holidays 8.00-16.00.

Start date 1 January 2027.
