Social and Health Care Assistant for Home Care Area West

The Home Care in Herning city, Area West, is seeking a social- and health assistant for a permanent position as of January 1, 2027, or by agreement. Planned holidays will be accommodated.

The position is 37 hours per week or as agreed, with weekend duty every 3 weeks.

We have base at Vestergade 92H together with the nurses, as well as close cooperation with the therapists in the area. We work with holistic care, and are organized in interdisciplinary teams.

As a social- and health assistant with us you will have an important central coordinating role around the interventions with citizens. You will also be an important sparring partner for students, substitutes, colleagues and partners in general. We are a training site, where we emphasize the importance of students having a good experience and becoming professionally proficient.

In addition, we have close cooperation with the dementia consultants in the homes where needed.

We have citizen conferences every week, where we elevate the interdisciplinary collaboration around the citizen. We work according to the Elderly Act focusing on the citizen experiencing as much coherence and self-determination in daily life. We work on moving from accountability to responsibility in our approaches.

We have ongoing opportunities for teaching/training together with the nurses.

We are working with "easy and faster access to APV assistive devices", and we have implemented screen visits for the citizens where professionally meaningful. We are a group that is very interested in Welfare Technology and would like to work in that direction.

We are about to start triage of each citizen, which we look forward to and see as a really good way to raise professional competence overall.

Our core task is personal care and practical assistance to citizens who live in their own homes.
