Two mini-teams - many professional disciplines - one common task! We are seeking an engaged social and health care assistant for the holistic care Centre 2 in Roskilde.

In Helhedsplejen Centrum 2 we are looking for a social- and health care assistant who wants to become part of our two mini-teams, where collaboration, professionalism and the citizen in focus are a natural part of everyday life.

In our mini-teams, social- and health care assistants and social- and health helpers work closely with nurses, therapists, dietitians, digi rehab, a virtual unit and other relevant professionals.

We have a daily routine where we prioritize meeting, coordinating and making each other look good.

That means, among other things, that we

• Start the day with morning coordination, where the whole team, as well as nurses and therapists, gather and create an overview of the day’s tasks, including triage.
• After the common morning coordination, nurses and assistants gather, where we ensure coherence and quality in the interventions.
• Have a once-a-week cross-disciplinary meeting, where all relevant professionals participate. Here we discuss the citizen’s needs/goals and we find the best solutions together.
• Work closely to support citizens’ independence, resources and quality of life.
• Are curious about new ways of working, and we are in the process of new workflows in the form of morning coordination, cross-disciplinary meetings and holistic care, and we want employees who are willing to contribute with ideas, professional sparring and engagement.

Working hours

Start time 7-13 and 7-15. Work every 3rd weekend.

32-37 hours per week.
