Social and Health Care Assistant for home care

Do you want to use your professional skills close to the citizens and be part of a team where continuity, quality and interdisciplinary collaboration are in focus?

We are looking for an engaged social- and health care assistant who wants to be part of our home care and contribute to safe and cohesive pathways for the citizens.

We are in an exciting development, where we, among other things, work with the implementation of the elderly reform. We have organized ourselves into smaller, geographically divided teams to create greater continuity, closeness and coherence in task solving.

At the same time we are working on implementing Deloitte route planning with a focus on creating a good alignment between the citizens' needs and the daily planning.

About us

You will be part of a fixed team, where we place emphasis on knowing the citizens, their needs and their daily lives.

We work closely across disciplines and meet weekly with our sister team for interprofessional conferences. Participants include, among others, case managers, nurses and therapists.

The conferences give us the opportunity to discuss citizens' situations, coordinate interventions and use each other’s expertise to create quality and continuity in citizens' care pathways.

We focus on professional sparring, development and a working environment where we help each other and tackle the tasks together.

Your role

As a social- and health care assistant you will play a central role in daily task execution and in ensuring quality and continuity for the citizens.

You work independently with your professional tasks, while being part of a team where we collaborate to find the best solutions for the citizens.

Your tasks will include

  • Nursing tasks for citizens in stable care paths
  • Holistic care and nursing based on the individual citizen's needs
  • Observation, assessment and documentation of citizens' condition
  • To respond to changes in the citizen's condition and initiate relevant actions
  • To contribute to rehabilitative interventions and professional assessments
  • To help ensure citizens' well-being, dignity and self-determination
  • Interdisciplinary collaboration with colleagues across disciplines
  • Participation in weekly interdisciplinary conferences
  • Opportunity for training in and handling of more complex nursing tasks
  • Receiving and handling calls from citizens
  • Illness and discharge notifications and helping to find good solutions in daily operations

We are looking for someone who

  • Is educated as a social- and health care assistant
  • Has high professional competence and is passionate about delivering high-quality care and nursing
  • Meets citizens and relatives with calmness, respect and professionalism
  • Can work independently and takes responsibility for your tasks
  • Can observe, assess and respond to changes in the citizen's condition
  • Thrives on interdisciplinary collaboration and sees the value in using each other's professional expertise
  • Is positive about changes and contributes constructively to the development of our workplace
  • Is flexible and eager to be an active part of a small team
  • Is familiar with IT and documentation – we work in Nexus and according to FSIII
  • Has a driver's license

We offer

With us you have the opportunity to use and develop your professional skills in home care, where we work purposefully with quality and continuity.

We offer

  • An engaged and professionally strong team
  • A geographical team structure that supports proximity and continuity for citizens
  • Close interdisciplinary collaboration with, among others, nurses, therapists and visitators
  • Weekly interdisciplinary conferences with opportunities for professional sparring and joint reflection
  • Opportunity for professional development and training in complex tasks
  • A workplace focused on well-being, collaboration and development
  • A thorough introduction so you get off to a good start
