We are seeking an engaged and professionally grounded social- and health care assistant for a permanent position as evening shift for our team in Middelfart.

As an assistant in the evening shift you will have an important role, where you, together with your colleagues, help to ensure safety, continuity and high professional quality for our citizens during the shift.

Your tasks

As a fixed evening shift, your tasks will include

• Responsibility for the health and care-related interventions during the evening shift
• To provide holistic care to our citizens
• Handling of acute calls and assessment of citizens in cases of sudden illness or fall
• Observation and professional assessment of changes in the citizens' condition
• Updating and documentation of the citizens' conditions and 24-hour rhythms
• To create calm, safety and continuity for the citizens.
• Collaboration with colleagues and relevant partners about the citizens' care journeys

We are looking for you who

• Are trained as a social- and health care assistant and are passionate about close contact with citizens
• Thrive working independently and can maintain an overview in pressured situations
• Have good clinical assessment skills and can act on changes in the citizens' condition
• Are responsible and work in a structured way
• Have good communication and collaboration skills
• Enjoy working in the evenings.
• Want to contribute to creating quality and safety for our citizens

Experience from home care is a requirement. For us it is also important that you have the motivation and professional competence to undertake the task.
