Social and Health Care Assistant for a Coordinator Role

Due to an organizational restructuring in the Elderly Care area in Favrskov Municipality we are seeking, as of December 1, 2026, a social- and health care assistant for a coordinator function in home care, Hadsten.

The position is a full-time position, where the coordinator function will account for about four hours of the daily working time. The remaining time you will be out in the field with tasks within holistic care.

You will also be required to have about 6 evening shifts over 12 weeks as well as a weekend shift every 4th weekend

As a coordinator you will, among other things, be responsible for

• the daily planning and coordination in the team you are attached to as well as overall coordination between all teams. This will involve planning of elder law processes as well as services under both the Health Act and the Service Act.
• to prepare driving lists that ensure continuity, and that tasks are solved with the right competencies
• to ensure stable operation, including coverage of shifts in cases of illness
• to answer calls from citizens, relatives and other partners

We attach great importance to interdisciplinary collaboration around the citizens and are organized into 3 fixed teams consisting of both nurses, social- and health care assistants, social- and health care assistants and unskilled workers.
