About the job You will become part of our mobile home care and nursing care, where we work in interdisciplinary and self-managing mini-teams.

Day shift

• Working hours: The position is full-time, but we are open to your desired number of hours. There will be a shift every 3rd weekend.
• Start time: 07:00-15:00
• Routes are carried out by bicycle and car, so you must have a driver's license

Evening shift

• Working hours: 5-2 rotation (Tuesday/Wednesday and even weekends)
• Start time: 15:00-23:00
• Routes are carried out by car, so you must have a driver's license

Your tasks

• Holistic care with a preventive and rehabilitative focus
• Follow-up on the nursing assessment after discharge
• Follow-up on the SUL interventions
• Be proactive in new professional instructions and guidelines

Professional development If you want more responsibility in the long run, there is the possibility to develop yourself in a resource function. For example, you might have an upcoming mobility instructor or an internship supervisor in mind? Or perhaps you are the one to facilitate citizen conferences?

It is an option, not a requirement.

About Lokalteam Midt We are about 56 employees in total, spread across social and health care assistants (SSH), social and health care assistants (SSA), nurses and therapists. We also train apprentices and students and prioritize learning, coaching and professional development highly – you should be willing to contribute to that.

At the same time, we have an informal tone and a good sense of community with social events throughout the year.

Who are we looking for? You must be educated as a social and health care assistant with Danish authorization. Experience from home care is an advantage but not a requirement.
