About the job

Permanent position as a health consultant at 37 hours per week with start on January 1, 2027

Do you want to help develop the health promotion and prevention of chronic diseases, rehabilitation and chronic disease initiatives in the local health care?

At Sorø Health Center we are looking for a health consultant who is passionate about creating value for people with chronic illnesses and who also wants to contribute to the development of the health initiatives that follow the health reform, the new quality standards and the upcoming chronic care packages.

With us you will have a central role in the interface between citizen-oriented interventions, patient pathways, quality development and cross-disciplinary and cross-sector collaboration.

Your tasks

You will among other things

  • conduct clarifying and motivational conversations with citizens and patients
  • teach and facilitate group programs
  • work with rehabilitation and disease management for people with chronic diseases
  • contribute to initiatives in COPD, smoking cessation and alcohol areas
  • collaborate with general practice, hospitals, civil society and patient associations
  • contribute to the implementation of new quality standards and chronic care initiatives
  • work with quality, documentation, evaluation and use of data
  • translate new knowledge into practice and develop future health offerings.

Organizational health competence

At Sorø Health Center we work with organizational health competence. We believe that it is not only about the citizen’s ability to navigate the health system, but also about our ability to make information, offers and pathways understandable, accessible and usable.

We therefore seek a colleague who can help develop offers that reduce barriers, strengthen health competencies and contribute to greater health equity.

We imagine that you

  • have a health professional bachelor degree
  • health education competencies
  • have a minimum of 3-5 years of experience with health promotion, prevention, rehabilitation and/or chronic disease areas
  • have STOP training from the Danish Cancer Society and knowledge of AUDIT in the alcohol area
  • thrive with both citizen contact and development tasks
  • have an interest in quality development, data and cross-sector collaboration
  • work independently while being a strong team player
  • are curious, relational and focused on creating coherence for citizens.

We offer

  • an exciting position in a health center with high professionalism and big ambitions
  • opportunity to influence the development of future prevention and rehabilitation initiatives
  • a strong cross-disciplinary community
  • a high degree of influence, development and responsibility
  • a workplace characterized by trust, psychological safety and good humor.

With us the question is not: "Why don’t citizens do what we expect?"

The question is: "How can we structure our offers so that more citizens succeed?"

If this sounds like a task for you, we look forward to hearing from you.
