About the job

Do you want to play a central role in ensuring the right effort for the right citizen/patient at the right time? Then we have an exciting combined position for you.

We are seeking a nurse or social- and health care assistant for a combined position with both clinical tasks in the Emergency Department and the role of SOP coordinator where you are the visitor for the Health and Care Placements in the Region of Syddanmark’s catchment area of Sygehus Lillebælt.

In connection with the health reform, we are strengthening and developing Sygehus Lillebælt’s joint visitation and are therefore looking for nurses and social- and health care assistants who, in addition to being passionate about the acute area, will help to visit citizens and patients to the regional Health and Care Placements.

We especially hope you will apply if you

  • have experience from municipal acute functions, rehabilitation pathways, municipal visitation or another coordinating function
  • have experience with patient-centered coordination, e.g. as discharge coordinator at the hospital
  • thrive in acute patient pathways and want to combine clinical work in the Emergency Department with a coordinating function in Sygehus Lillebælt’s Joint Visitation

The task as SOP coordinator

For Health and Care Placements in the four municipalities Sygehus Lillebælt collaborates with, citizens and admitted patients who need health care interventions that cannot be managed at home and who do not require specialized treatment and care at the hospital are visited.

As a visitor to Health and Care Placements your tasks will include

  • coordinate the best possible course for citizens and patients in need of a Health and Care Placement
  • obtain and assess information about the citizen’s/patient’s prior course, home conditions and need for health care interventions
  • be the link between hospital, municipality, GP and Health and Care Placement up to final visitation
  • be investigative and curious in cases where the citizen/patient does not meet the visitation criteria – and here uncover alternative offers and possibilities.

Your workday will be characterized by coordination and collaboration across departments at the hospital and across hospital and municipality – combined with clinical work in the Emergency Department.

Working in the Emergency Department

Our core task in the Emergency Department is to receive acute patients, treat and care for them in collaboration with the doctors, and prepare for further pathways, which may be transfer to a department, surgery or discharge home or to municipal interventions.

You will become part of an environment where tasks are varied and the pace is often high and where collaboration and coordination across is a natural part of daily life.

Onboarding

We have a strong onboarding process in our learning laboratory over 4-6 weeks. Here a small group of experienced and engaged onboarding nurses will introduce you thoroughly to the work in the Emergency Department.

Introduction to the function as SOP coordinator takes place after completing the introduction to the Emergency Department, so you gain solid knowledge of the acute area as a basis for the visitation task.

We offer

  • a stimulating combination of clinical nursing and coordination tasks
  • a central role in creating cohesive and safe pathways across sectors.
  • working hours in visitation are expected to be 08:00-16:00
  • evening/night, weekend and holiday shifts in the Emergency Department.
  • close inter- and multi-disciplinary sparring with colleagues in visitation. Here you will be 4-5 staff present during the day, including nurses, secretaries and doctors linked to Sygehus Lillebælt’s Joint Visitation.

We are looking for you who

  • have knowledge, insight and understanding of both the municipal and regional area
  • have experience from or extensive knowledge of municipal visitation, acute function, rehabilitation pathways or other coordinating function e.g. as discharge coordinator at the hospital
  • have knowledge of, and understanding of, offers and possibilities in both sectors – we work from the principle: “no rejection without recommendation”
  • have the ability and willingness to work across departments, professional groups and sectors
  • can work independently, create overview, prioritize and act, and at the same time function well in team collaboration
  • have understanding of and insight into good and cohesive patient and citizen pathways
  • thrive in a busy environment and can see yourself in shifts where up to 50% of shifts are either evening or night shifts as well as weekends and holidays

If you would like to coordinate the good pathways for our shared patients and citizens and you see yourself in the acute specialty? Then we look forward to receiving your application.
