About the job

Be part of creating safe and coherent citizen journeys in Svendborg Do you want to use your professional expertise in a strong multidisciplinary citizen journey and at the same time help shape a new regional health offering?

As a social- and health care assistant with us, you will play an important role in citizens' journeys. You meet people who for a period need health care treatment, care and rehabilitation – with the common goal of creating safe and coherent pathways and supporting that the citizen can return to everyday life.

At the same time you will have the opportunity to influence workflows, professionalism and collaboration in a new regional offering.

You will become part of something new

Borgernær Sundhed is a new department at Odense University Hospital, which will strengthen local health services and bring treatment, care and rehabilitation closer to citizens' daily lives.

From January 1, 2027, Borgernær Sundhed takes over responsibility for the regional health and care beds in Svendborg

Here, a service with initially 16 beds is established. The goal is to help the citizen back to their own life as best as possible through dignified, coherent and professionally strong pathways.

We are in a establishment phase, where employees with different professional backgrounds must develop common workflows and create a strong professional environment. You therefore do not join a finished setup, but will have the opportunity to actively contribute your experience and professional expertise from the start.

Your role as social- and health care assistant

You will become part of a multidisciplinary work community with social- and health care assistants, nurses, physiotherapists, occupational therapists and doctors.

As a social- and health care assistant you will have a varied daily life with both independent responsibility and close professional sparring.

Your tasks

You will, among other things, come to

  • observe, assess and respond to changes in the citizen's health condition
  • perform health-care nursing and treatment tasks within your competence area in close collaboration with the nurse.
  • medication management with systematic follow-up
  • contribute to assessment, clarification and rehabilitation
  • create safety and coherence for the citizen and their relatives
  • document the citizen's pathways and the health care interventions in EPJ-Syd
  • contribute to professional development, quality and patient safety

We place emphasis on you

  • are professionally curious and want to develop yourself
  • have a strong professional competence
  • work independently and at the same time thrive in a multidisciplinary work community
  • meet citizens and relatives with respect, calm and presence
  • see rehabilitation as a natural part of care and treatment
  • contribute constructively to collaboration and the work community
  • thrive in an organization under development, where not all workflows are predefined in advance.

Experience with complex citizen pathways, rehabilitation, the acute area or the municipal or regional health care system is an advantage but not a prerequisite.

With us you get

  • opportunity to use and develop your professional competencies
  • influence on workflows and professionalism from the start
  • a daily life with focus on quality, development and patient safety
  • an individually tailored onboarding program
  • a workplace where we help each other and actively use each other's different professional skills.

Terms of employment

The position is as a rule 37 hours per week with mixed shifts. It will however be possible to have fixed evening or night shifts.

Employment takes place in accordance with the applicable collective agreement.
