What will your daily life look like?

You will be an important part of the citizens' daily lives and of our shared task to create safe, dignified, and cohesive citizen journeys.

Your tasks will include, among others,

• Providing personal care and practical help based on the citizen's needs and wishes.
• Working rehabilitatively and supporting the citizen's independence.
• Contributing to holistic care and continuity in the citizen's process.
• Observing changes in the citizen's condition and acting accordingly.
• Documenting and communicating professional observations.
• Collaborating with colleagues and interprofessional partners.
• Contributing to development, quality, and professional sparring within the team.

As a social and health care assistant you will also have the opportunity to take responsibility for your own citizen care trajectories and help coordinate the assigned interventions.

Experience with dementia care is an advantage, but not a requirement. The most important is your professionalism, your motivation, and your desire to make a difference. If you have experience with Digi-rehab or screen visits, we would also like to hear about that.

Will you join the team?

We are seeking two social and health care assistants and one social and health care helper for primarily day shifts at 37 hours per week, with the possibility to agree on a lower number of hours.
