We are still looking for engaged colleagues who want to help shape a new department, create a strong professional community and make a noticeable difference for citizens every single day. You will work in new and updated physical surroundings, and you will be an important player in building our new department.

We are seeking 2 authorized social and health care assistants who are passionate about complex citizen pathways, rehabilitation and interdisciplinary collaboration in close health care.

The positions are initially 37 hours per week; if you would prefer a lower number of hours, there is also an option for that.

You will be included in a duty roster with weekend shifts. You can obtain a day shift position, where you also have evening or night shifts, or a position in a fixed shift type

Sundheds- og omsorgspladser Vejle The staff group consists of social and health care assistants, nurses, occupational therapists, physiotherapists, housekeepers, service assistants and medical cover from hospital and general practice.

Our core task is to receive citizens with complex needs for specialized nursing, rehabilitation, and palliation of a nature that cannot be solved at home, and a main task is to bridge the gaps between sectors.

Citizens shall be rehabilitated through a close interdisciplinary collaboration so that citizens are strengthened regarding resources and health competencies, and thereby regain their independence and a higher quality of life.

There will be close cross-sector collaboration with acute nursing in Nær Sundhed, general practitioners, hospital departments responsible for treatment, training as well as municipal assistance and nursing.

The goal is that multi-ill citizens safely return home, and that readmissions are prevented.

About the position As a social and health care assistant at the Health and Care Pladser in Nær Sundhed, the core task is to ensure that citizens with complex health needs receive a coherent, professionally qualified and secure service in the aftercare or as an alternative to hospitalization. Our task is to strengthen functional ability and well-being for the individual citizen.

You will follow a structured and well-planned introduction program and will have good opportunities to develop your competencies.

Who are you? You are a social and health care assistant.

You like playing a central role in the daily contact around the citizen, where the following is included:

• Observation, care and treatment of citizens with various and complex health needs.
• Rehabilitation based on the citizen's own goals and resources.
• Medicine management and systematic follow-up on the citizen's condition.
• Close interdisciplinary collaboration with nurses and therapists.
• Dialogue and collaboration with the citizen and relatives.
• Documentation and knowledge sharing across professional groups.
• Planning and coordination of coherent pathways and transitions to home.
• Participation in work with quality, patient safety and development of our practice.

About the department The Health and Care Pladser in Vejle are located at Brummersvej 1 A, 7100 Vejle, and are part of “Nær Sundhed” - a new department in Sygehus Lillebælt, which is under construction.

Salary and employment terms Salary and terms of employment for the position follow the Social and Health Care Personnel – Collective Agreement 32.03.1 - salary as of 1.4.2026. In addition to the starting salary, the pay may include negotiated allowances depending on the position, your experience and qualifications.
