About the job

Do you want to help create coherence, mastery and new opportunities for adults in vulnerable positions?

Are you passionate about relationship work, social support and helping people build a more stable and independent life? Do you thrive in a flexible everyday life where collaboration, bridging and prevention are a natural part of the work?

Then you now have the opportunity to become part of the Resting Places (Væresteder) and Special Initiatives (Særlige Indsatser) in the Center for Disability and Psychiatry in Frederikshavn Municipality.

We are looking for two engaged and pedagogically educated staff members who will help strengthen our efforts for adults in vulnerable positions. One position is as a case manager in Housing First, while the other is a combined role, where you work as a case manager in Housing First and as a staff member in The Social Pit Stop (Det Sociale Pitstop).

About Resting Places and Special Initiatives

Resting Places and Special Initiatives comprise a range of different offers and initiatives aimed at preventing social vulnerability and supporting citizens' opportunities for coping, independence and participation in meaningful communities.

The area includes the Resting Place in Skagen, Bakkely and the House in Skansegade in Frederikshavn, Clasens Gård in Sæby, The Social Pit Stop, the Housing First team, the § 99 outreach contact person scheme and the Peer Education program.

We work across professional boundaries and collaborate with municipal and regional actors, civil society and local communities. We focus on creating coherent interventions that start from each citizen's needs, wishes and resources.

About Housing First

Housing First is based on a fundamental understanding that having one's own home is an important foundation for creating stability and development in life. Based on the citizen's wishes and needs, we work to support housing retention, recovery, coping and independence.

As a case manager you will be the citizen's ongoing contact person and fixed anchor. You work relationally, flexibly and outreach-oriented – often in the citizen's own home – and support the citizen in creating structure and stability in daily life.

A central part of the task is to coordinate interventions and bridge the gap between the citizen and relevant partners, including authorities, psychiatry, addiction treatment, the employment sector, civil society and local communities.

About The Social Pit Stop

The Social Pit Stop is the open entry point in Frederikshavn Municipality across the municipality, where adults in a difficult life situation can seek help without referral or prior assessment. Here we offer early and preventive support based on the citizen's needs and with a focus on the least intrusive intervention. This can include counseling, group-based interventions and bridging to relevant services, civil society and local communities. The Social Pit Stop is present in Skagen, Frederikshavn and Sæby.

The two positions

Case Manager in Housing First

As a case manager you will be responsible for supporting citizens in Housing First to establish and maintain their own housing and create a more stable daily life.

Your tasks will include, among others

  • Build and maintain trusting relationships with the citizens based on their wishes, needs and motivation.
  • Provide flexible and outreach-oriented support, often in the citizen's own home.
  • Support housing retention, including daily structure, practical matters and handling challenges that may threaten the housing.
  • Coordinate interventions and create coherence in collaboration with relevant actors.
  • Bridge to treatment, employment, networks, civil society and local communities.
  • Document the intervention systematically and follow up on the citizen's goals and progression.

Case Manager in Housing First and Staff Member in The Social Pit Stop

In this combined role you will be part of the Housing First team and also spend part of your work time in The Social Pit Stop. The position includes shifts in The Social Pit Stop across Skagen, Frederikshavn and Sæby.

Your tasks will – in addition to the Housing First case manager tasks – include, among others:

  • Engage with citizens and engage in relationship work with adults in vulnerable positions.
  • Contribute to and implement preventive interventions, e.g. counseling, group activities and support under the Social Services Act § 82 b.
  • Support citizens' participation in communities and group-based interventions.
  • Bridge to civil society, local communities and other relevant services and partners.

We are looking for a staff member who thrives with individual citizen pathways, and who also recognizes the preventive possibilities in communities, networks and cross-sector collaboration.

Who are you?

We imagine that you

  • Are pedagogically educated and have experience with social work with adults in vulnerable positions.
  • Have experience with relationship work and building trusting collaborations with citizens who may have complex issues.
  • Have experience with Housing First or knowledge of the target group for the initiative. Experience with Housing First methods CTI, ICM or ACT/M-ACT is an advantage.
  • Can work independently and in a structured way while participating in close interdisciplinary collaboration.
  • Are flexible and persistent and can adapt your approach to the individual citizen's situation and pace.
  • Have an eye for the citizen's resources, wishes and possibilities and work with a focus on recovery, coping and independence.
  • Can document your work systematically, and preferably have knowledge of the case system CURA.

For the combined position we also place emphasis on having an interest in preventive interventions and collaboration with civil society and local communities. It is an advantage if you have experience with or interest in group facilitation and group-based offerings.

We offer

  • A meaningful job where you can make a concrete difference for adults in vulnerable positions.
  • An workplace with engaged colleagues and focus on professional sparring and interdisciplinary collaboration.
  • Opportunity to put your professional competencies to use and contribute to qualifying our interventions.
  • A daily life with variety, independence and the possibility to organize the work flexibly.
  • Opportunity to work with both individual pathways, prevention and bridging to communities and relevant offers.
