Are you a social- and health care assistant, and are you motivated by complex citizen journeys, high professionalism and interdisciplinary collaboration? Do you want to help create safe and coherent pathways for citizens with complex health and care needs? Then maybe you are the one we are looking for.

The Health and Care Places consist of 20 beds and are an important part of the future of local health services. Here we receive citizens with complex health- and care needs who require observation, treatment, rehabilitation and support in the transition between hospital, municipality and their own home.

As a social- and health care assistant you will play a central role in the daily efforts around the citizens. You work closely with the citizen and use your professional competencies for observation, care, rehabilitation and coordination of efforts in collaboration with nurses, therapists and other partners.

Practical information

Employment from January 1, 2027 or by agreement.

The positions are on 37 hours per week and involve work in mixed shifts. You must be able to participate in a minimum of two shift teams. There is SSA coverage in day, evening and night shifts, and weekend work is included according to the current rota.

Salary and terms of employment according to the applicable collective agreement for social- and health personnel entered into with FOA.
