About the position

In the Integrated Care, we work to create coherent, holistic and individually tailored pathways for citizens. We base our approach on each citizen's needs, resources and wishes and work purposefully to support quality of life, self-determination and safety in the citizen's own home.

As the evening shift team leader you will play a central role in ensuring that the evening shift functions professionally, organizationally and humanely. You will become an important sparring partner for the staff and must contribute to ensuring that both daily operations and nursing professional quality are in focus.

You will have the opportunity to combine your nursing expertise with coordination, professional sparring and leadership close to practice.

Your primary tasks

• To ensure high quality in the professional work and coherent care for the citizens.
• To support and coordinate daily operations in the evening shift.
• To be a visible, accessible and present leader for the staff.
• To provide nursing professional sparring and support the staff's professional development.
• To prioritise and coordinate efforts when citizens' needs change or acute situations arise.
• To contribute to good interdisciplinary collaboration with colleagues, partners, citizens and relatives.
• To support documentation, patient safety and professional quality in accordance with current guidelines.
• To work with development, implementation and anchoring of new workflows and initiatives in the Integrated Care.

About you

• Are a trained nurse with solid nursing expertise.
• Have an interest in leadership, coordination and development of staff and workflows.
• Preferably have experience with home care, municipal nursing or similar.

• Can maintain an overview and make professionally well-founded decisions in a changing daily life.
• Work in a structured way and can prioritise tasks with a view to both citizens' needs and staff resources.
• Are solution-oriented and actively contribute to a good working environment.

Employment: 37 hours Working hours are primarily 15:00 – 23:00, but with meetings, sparring etc. during the day.
