Visiting nurse for Holistic Care – Langebæk East

In the Home Care (Helhedsplejen) in Vordingborg Municipality we work to create coherent citizen journeys and strong interdisciplinary teams around the citizens. As a nurse you will play a central role in ensuring coordinated integration of both home care and nursing for the citizen.

About the job

As a visiting district nurse in Langebæk Øst you will be an important part of the holistic care, where you deliver nursing of high professional quality in the citizen's own home. You meet citizens with complex and varying needs and work independently with clinical assessments, treatment, guidance and coordination of coherent citizen journeys.

You will become part of a professionally strong team where collaboration, sparring and the citizen’s quality of life are at the center. The workday is varied, and you will have great opportunity to apply your nursing knowledge in close cooperation with citizens, relatives, colleagues and other partners.

Your main tasks

• Carry out nursing interventions for citizens in their own homes.
• Conduct clinical observations, assessments and follow-up on citizens' health status.
• Coordinate and ensure coherent citizen journeys in close collaboration with home care, therapists, general practitioners and hospitals.
• Handle medication management, wound care, sample collection and other nursing tasks according to current guidelines.
• Contribute to triage and early detection of illness and prevention of hospitalizations.
• Inform and teach citizens, relatives and colleagues on health-related issues.
• Document and ensure high quality in the nursing efforts.
• Contribute actively to professional development, interdisciplinary collaboration and a good working environment.

Who are you?

We imagine that you

• Are a trained nurse, preferably with medical or psychiatric experience
• Can create structure, prioritize and maintain an overview, even when daily life changes
• Have a good understanding of the competencies of different professional groups
• Communicate clearly and collaborate well with both staff and management
• Have good IT skills
• Have some knowledge of Nexus, but it is not a requirement.

Practical information

The position is 37 hours and every 4th weekend is worked.

The working hours will be day shifts.
