Visiting nurse with a specialization in COPD

We are seeking a nurse who may have at least 2 years of experience within medical specialties. It is an advantage if you have experience in observation, diagnostic assessment, treatment, rehabilitation and palliation for citizens with COPD.

The position is a full-time role with hours shared. You will work as the municipality's specialist 16 hours per week, covering the entire Norddjurs municipality. You will help develop and implement best practice in the COPD area and we assess citizens in their own homes, at care homes in close collaboration with the nurses in the individual units. The remaining hours, including during the vacation season, are delivered in the Allingåbro home care nursing.

The position is anchored administratively with management in Allingåbro. The professional leadership of the specialty is handled by our contract holder for the specialty area.

Purpose of the role as COPD nurse

• As a specialist, you will help ensure that citizens receive the right health care services that meet the national care pathway for COPD
• Train and develop the organization's professional competencies within COPD
• Help prevent hospital admissions and readmissions
• Support the citizen's own ability to manage life with COPD.
• Help improve citizens' quality of life
• Educate and disseminate knowledge about COPD to citizens, relatives and partners

We are seeking a nurse who

• Wants and can deliver nursing care and advice of high professional quality
• Can be inspiring and good at building relationships
• Focus on citizens' own resources and have a rehabilitative approach in close relationship with the citizens
• Is good at collaborating across sectors and multidisciplinary
• Has good knowledge of COPD, and a desire to develop this.
