About the job

We have the opportunity to hire several regional visitators for a new function at Bispebjerg and Frederiksberg Hospital

Do you have strong clinical experience, a good overview of the overall patient journey and a desire to work across the hospital and municipality? Then you may be the one we are looking for.

From January 1, 2027, the regions will have a new task with visitation to health and care places. At Bispebjerg and Frederiksberg Hospital we are therefore establishing a new regional visitation function, and we are looking for several visitators who will help build and develop the function.

A common function across the hospital

As a regional visitator you will be part of a common function for the entire hospital, organizationally placed in the Geriatric and Palliative Department, where you will also receive your daily management.

In the municipalities of Copenhagen and Frederiksberg there are in total 239 health and care places spread across several sites, to which the regional visitation function will perform visits.

Your task will be to assess patients who no longer need hospitalization, but are also not ready to go directly home, and decide whether a health and care place is the right offer.

You will among other things

  • visit patients to health and care placements.
  • assess the patient’s overall health, functional and rehabilitative needs.
  • match the patient with the relevant offer and have an overview of capacity.
  • coordinate the transition from hospital to a health and care placement.
  • collaborate with hospital departments and municipalities and follow up on patient pathways.

You will thus be an important bridge between hospital, municipalities and health and care placements.

We will develop the function together

The function is new, and you will have the opportunity to influence workflows, collaborations and professional practice from the start.

We are in the process of building the new team and are looking for more colleagues who want to take responsibility, contribute with their expertise and help find good solutions to a new and important task.

The task requires you to balance the needs of the individual patient with a view to capacity and flow. You must be able to create an overview of complex pathways, collaborate across sectors and make independent visitation-technical assessments.

Who are you?

You have a health professional education such as nurse, physiotherapist or occupational therapist

It could for example be experience from hospital, municipality, rehabilitation, geriatrics, medicine, acute care or other areas with complex patient and citizen pathways.

It is an advantage if you have experience with discharge, coordination, cross-sectoral pathways or municipal offers. Experience with visitation is relevant, but not a requirement.

We especially emphasize that you

  • have a solid health professional perspective and can assess the needs of complex patients.
  • can create an overview and make decisions on a professional basis.
  • work independently and in a structured manner.
  • communicate clearly and have strong relational skills.
  • have an interest in flow, capacity and organizing.

We offer

  • the opportunity to help build and shape a completely new regional function.
  • a multidisciplinary team with different health professional perspectives.
  • close collaboration with hospital departments and municipal partners.
  • a varied workday with independent responsibility and potential for professional development.

Practical

The position is initially 37 hours per week. On special requests, other arrangements can be made.

Working hours will generally be during weekdays in daytime hours. The function must, however, be available all year round, and there will therefore also be weekend and holiday shifts. Weekend shifts will be shorter than regular weekday shifts and are expected to occur roughly every 7th weekend and on some holidays.

Salary and employment terms follow the applicable collective agreement.

Start: December 1, 2026 or as soon as possible thereafter.

Would you like to be part of it?

If you want to use your health professional experience in a new way and help create good transitions between hospital and municipality, we would very much like to hear from you.
