We are looking for an engaged colleague who wants to help shape a new department, create a strong professional community and make a noticeable difference for citizens every single day. You will work in new and updated physical settings, and you will be an important player in building our new department.

We are seeking an authorized nurse who is passionate about complex citizen journeys, rehabilitation and interdisciplinary collaboration in the near health care system.

The position is, in principle, 37 hours per week; if you would like a lower number of hours, there is also an option for that. You will be included in a duty roster with weekend shifts. You can obtain a day shift position, where you also have an evening shift, or a position in a fixed evening shift.

Sundheds- og omsorgspladser Vejle The staff group consists of social and health care assistants, nurses, occupational therapists, physiotherapists, house assistants, service assistants, as well as medical coverage from hospital and general practice.

Our core task is to receive citizens with complex needs for specialized nursing, rehabilitation and palliation of a nature that cannot be solved at home, and a main task will be to build a bridge between sectors.

Citizens must be rehabilitated through close interdisciplinary collaboration, so that citizens are strengthened in terms of resources and health competencies, and thereby regain their independence and a higher quality of life.

There will be close cross-sector collaboration with acute nursing in Nearby Health, general practitioners, treatment-responsible inpatient wards, training, and the municipal help and nursing care.

The goal is that multisick citizens come safely home to their own home, and that readmissions are prevented.

About the position As a nurse at the Health and Care Places in Nearby Health, the core task is to ensure that citizens with complex health needs receive a coherent, professionally qualified and safe intervention in the follow-up after discharge or as an alternative to hospitalization.

The core task is to strengthen the functional ability and well-being of the individual citizen.

About the department The Health and Care Places in Vejle are located at Brummersvej 1 A, 7100 Vejle, and are part of “Nearby Health” - a new department in Sygehus Lillebælt, which is under development.

In addition to the starting salary, the pay may include negotiated allowances depending on the position, your experience and qualifications.
