About the job

For us, visitation is not about finding an available slot. It is about ensuring the right intervention for the right citizen at the right time.

As a result of the Health Reform, the North Jutland Region will, under the Health Council Limfjord, take on the responsibility for health and care placements and the authority for acute nursing care. The Visitation Unit will therefore play a central role in ensuring cohesive citizen pathways across municipalities, hospitals and general practice.

Therefore we are now seeking visitation nurses for the new joint Visitation Unit in the Department of Citizen‑Centered Health.

As a visitation nurse you will play an important role in the citizen’s course. You will create overview, conduct clinical holistic assessments and navigate a complex health system with many actors, perspectives and interests. At the same time you will become part of the visitation unit from the start and have the opportunity to shape the development of work routines, collaboration interfaces and professional practice.

You will be responsible for assessing, coordinating and routing citizens to the intervention that best matches their needs. You will become a key person in the collaboration between municipalities, hospitals, general practice, the on-call doctor service and the Pre-hospital Care Company, where your professional judgments will influence both citizen pathways and the use of health system resources.

The position requires clinical depth, professional judgment and the ability to navigate safely across sectors. In return, you will have the opportunity to use your professional expertise every day to create value for citizens with complex health-care needs and contribute to ensuring they receive the right care at the right time.

Your tasks will include among other things

  • Route to health and care placements and acute nursing care
  • Carry out clinical holistic assessments of citizens with complex health-care issues
  • Coordinate citizen pathways and ensure secure transitions between sectors
  • Support capacity management with a focus on the best solution for the citizen
  • Collaborate closely with municipalities, hospitals, general practice, on-call doctor service and the Pre-hospital Care Company
  • Contribute to the development of work routines, visitation criteria and quality improvements
  • Provide professional sparring to partners and colleagues

You will work under medical delegation and have access to both visitation guide, decision support and ongoing medical professional sparring.

To maintain the close connection to practice and the citizen pathways that visitation supports, you will also participate in clinical work at the health and care placements.

We imagine that you are a nurse with solid clinical experience and a strong professional foundation. That you have experience with complex citizen or patient pathways and enjoy creating overview, coordinating interventions and finding solutions across organizational and professional boundaries.

We expect that you have solid knowledge of the interaction between municipality, hospital and general practice. Experience with visitation will be an advantage. You understand the different organizational conditions and can navigate a complex health system with many actors and perspectives. At the same time you have the clinical depth, professional judgment and impact to stand up for your assessments and make qualified decisions for the benefit of the citizen.

We particularly emphasize that you

  • Have strong clinical competencies and professional gravitas
  • Can safely navigate complex citizen pathways
  • Understand the interaction between municipality, hospital and general practice
  • Can build strong collaborative relationships across sectors
  • Can maintain an overview when many actors are involved
  • Are clear in your communication and stand firmly by your professional judgment
  • Work independently, structured and take responsibility for your decisions

We offer a professionally strong environment where high professionalism, integrity, trust and collaboration are the foundation for solving tasks. We help each other, challenge each other professionally and have high ambitions on behalf of the citizens.

You will become part of a new visitation unit with a central role in the future citizen-centered health system and have the opportunity to work at the intersection of clinic, coordination and cross-sector collaboration. At the same time you will gain access to a strong cross-disciplinary community and close medical professional sparring.

The Department of Citizen‑Centered Health includes among others health and care placements, acute nursing care and medical treatment responsibility. The Visitation Unit will be a central turning point in the work of creating cohesive citizen pathways and ensuring that citizens receive the right help at the right time.
