At Plejecenter Porsebakken we offer a safe and meaningful everyday life for our 48 residents, who live spread across three departments. The residents receive the daily necessary support and assistance to ensure a good and dignified existence. We are a multidisciplinary team consisting of nurses, social- and health-care assistants, social- and health-care assistants as well as house assistants. The house also houses many students and cross-disciplinary therapists.

About the position

As a team coordinator at Porsebakken you will as a starting point be attached to one fixed department, where you in close cooperation with your colleagues and your nurse colleagues plan and coordinate the day's tasks. In collaboration with your colleagues in the department you will be responsible for ensuring effective patient-safe treatment and care for the residents.

Your tasks will include

• Accountable for residents' nursing-related issues
• Coordination and planning of nursing, care and treatment for residents in the department
• Coordination of resident pathways with focus on coherence in care and rehabilitation efforts
• Communication of current nursing care
• Ensure that the documentation meets applicable requirements and standards
• Execution of nursing tasks for residents in stable courses
• Ward rounds weekly with the house doctor

We are looking for an SSA who can maintain an overview, has strong empathy and strong interdisciplinary collaboration skills. Someone who can see themselves in the many varied tasks and will help ensure a high professional level of care and nursing.

Additional information

The position is day shift for 37 hours per week - but we are flexible, so please let us know what you dream of. The position includes weekend duty every 3rd weekend, but can be planned in a 2-4 arrangement.

Salary and employment terms according to the applicable collective agreement and in accordance with the Ny Løn principles.
