About the job

About the job

Are you passionate about clinical nursing, citizen education and rehabilitation for citizens in their own homes? Do you enjoy long-term citizen journeys where you can build a good understanding and a close relationship with citizens and relatives? And would you like to use your nursing expertise across the entire care with a view to the citizen's overall life situation?

Then you may be the new colleague we are waiting for!

We are seeking a nurse who, together with the team's four other nurses, two social- and health-care assistants in day shifts and the rest of our multidisciplinary team, will help strengthen coherence and quality in citizens' pathways.

Nursing with a view to the whole person

In Team Hjortshøj we want to develop our practice based on Fundamentals of Care. That means we see the relationship with the citizen and the connection between physical, psychological and social needs as central parts of nursing. We take the citizen's wishes, resources and life situation as our starting point and work together to create a daily life that makes sense for the individual.

Therefore we are now expanding our nursing group. We want to bring nursing expertise even closer to citizens' daily lives and strengthen the overall care, clinical assessment and ongoing follow-up in collaboration with the team’s other professional groups.

With us, your work includes both specific nursing tasks and basic care, including personal care. You use your clinical eye in meeting the citizen, for example during support for personal hygiene, meals and mobility. Here you can build trust, support the citizen's own skills and notice changes in health and function.

We are therefore looking for you who see holistic care as a natural and meaningful part of your nursing profession, and who want to translate this approach into practice together with us.

Coherent and rehabilitative citizen pathways

In Care and Rehabilitation Team Hjortshøj we want to create the best conditions for citizens to experience coherent and well-coordinated pathways based on their wishes, needs, resources and potentials.

We provide nursing, personal care, practical help, training and rehabilitation for citizens who need support over a longer period. Citizen pathways range from less complex to complex pathways for chronic illness, multimorbidity and citizens in the final period of life.

We work rehabilitatively and multidisciplinary with a focus on health promotion, prevention, citizen education and citizen safety. We work with the basis in the Health and Care Pathway Model.

In our multidisciplinary and monodisciplinary meetings we share knowledge, spar on citizens' pathways and develop our competencies. The multidisciplinary meetings take place in the team, while the monodisciplinary meetings take place with the nearest colleagues in the network and across the district.

About Team Hjortshøj

In our team there are employed social- and health care assistants, social- and health care assistants, nurses, occupational therapists and a physiotherapist as well as a nurse and a physiotherapist focusing on competence development. We have skilled and experienced staff, both qualified and unqualified, who contribute to citizens' everyday life.

Together we work rehabilitatively with citizens' goals and overall situation. You contribute with your nursing knowledge, participate in the care and support your colleagues' professional development through sparring and shared reflection.

We prioritise ongoing professional development, high professionalism, good well-being and a good working environment, which everyone contributes to. As a nurse you will have the opportunity to shape development in the team in collaboration with management and your colleagues.
